These forms must be printed, filled out, and sent to Special Olympics Greater Memphis office for any individual who will be training as a Special Olympics Athlete or Unified Sports® Partner.

 

Volunteer

This Volunteer Application must be filled out in its entirety and mailed to Special Olympics Tennessee

The following description of a Class A volunteer can be found in Section 7.13 (a) (1) of the Special Olympics Official General Rules.

“Class A” volunteers are those who (i) are or may be in regular close physical contact with athletes, such as coaches, drivers, and overnight hosts, or (ii) have or may have administrative and/or fiscal authority.”

Because much of the information requested on the application is confidential information we ask for no faxes please.

Atlantoaxial Condition

Medical research has determined that up to 15% of individuals with Down Syndrome suffer from a condition known as Atlantoaxial Subluxation (Dislocation), which is a malalignment of cervical vertebrae C-l and C-2 in the neck.
