Unified Partner® Release Form
This form must be printed, filled out, and sent to your local area director for any individual who will be training as a Unified Sports® Partner on a Special Olympics Team. The Unified Sports® Partner Release form must be on file with the local Chapter office prior to the occurrence of sport training.
It is important to remember that Unified Sports® Partners (age 18 and above) are technically considered "Class A" Volunteers and therefore are subject to the same policy requirements as other delegation volunteers.
